Mega Sports Camp Registration 2025

This year's theme for Mega Sports Camp (Vacation Bible School) is In Your Element! In Your Element is designed for kids to engage with the science that supports sports. They'll learn what it takes to be a better athlete and to grow as a follower of Jesus. All students from 1st to 5th grade are invited to join us.
Our camp will take place at St. John (5171 Milford Road East Stroudsburg, PA) from June 23-June 27, 9am to 12pm.

Please read this waiver and sign below.Mega Sports Camp Liability Waiver I, the undersigned parent/guardian, do hereby grant permission for my son/daughter, named on this form: (named above)to attend Mega Sports Camp. My child will be (age listed above) during camp week. I fully understand that there is always a possibility that my child may sustain physical illness or injury while playing sports. I acknowledge and understand that my child is assuming the risk of such physical illness or injury by his/her participation, and I further release the sponsoring organizations (Church of St. John, East Stroudsburg, PA) and its representatives from any claims for personal illness or injury that my child may sustain during the camp. I further acknowledge and understand that my child will be responsible for his/her failure to abide by the rules and regulations of the camp. If he/she needs medical attention, that is, a bandage, ice, etc..., I authorize the safety coaches/camp nurse to help my child including calling 911 to obtain emergency assistance. If other injury occurs, I understand that the camp directors will attempt to contact me at the emergency numbers I have provided on the registration form and here, and that I am asked to be reachable at all times at the numbers provided. However, if I am unreachable, I hereby authorize the camp staff and representatives to act on my behalf should an injury occur. To ensure my child may receive the proper medical treatment in the event that he/she may sustain injury or illness during MEGA Sports Camp, I hereby authorize the camp staff to obtain or provide medical treatment for my child for such injury or illness during the camp, and I hereby hold the camp staff and sponsoring organizations, as well as its representatives, harmless in the exercise of this authority. I further acknowledge and understand that I will be responsible for any medical bills that may be incurred on behalf of my son/daughter for physical illness or injury that he/she may sustain during camp.
